This notice is a pre-launch draft. Items marked [TO CONFIRM] must be completed by Lavuna before launch.
There are only two things this site collects, and they are kept apart. The survey is anonymous: six answers and nothing else — no name, email, phone number, date of birth or address.
If you join the early-access list, we store your email address on its own. There is no identifier connecting it to any survey response, so the two can never be joined up.
We use early-access emails only to tell you when Lavuna is ready to try. You can ask us to remove your address at any time.
Lavuna does not provide dental diagnoses and does not replace professional dental advice.
Who is responsible for this site and your information
This site is operated by the team creating Lavuna, [TO CONFIRM: registered company name], at [TO CONFIRM: registered address], company number [TO CONFIRM: company registration number]. Lavuna is being developed and is not yet available to use.
We are the data controller for the information described here. You can reach us at info@lavuna.com.
What information we collect
Two separate sets of information, and nothing else. The anonymous survey collects answers with no email address attached. The early-access form collects an email address with no survey answers attached.
Anonymous survey responses
If you complete the survey, we store your answers to the six questions, including any free text you choose to write, and the date and time the response was received.
No name, email address, phone number, date of birth or address is collected or stored with survey answers. There is no account, no login and no identifier that links a survey response back to you or to an email address. Because responses are anonymous, we cannot find or remove an individual response later.
Free-text answers are the one place personal details could appear, because you write them yourself. Please only include what you are comfortable sharing anonymously.
Early-access email addresses
If you join the early-access list, we store the email address you give us, the fact that you asked to be contacted, the date and time, and which part of the site you signed up from. If you arrived through a campaign link, the campaign parameters in that link are stored too; when there are none, those fields are left empty. Nothing else is recorded, and this is held separately from survey responses.
Each email address appears on the list once. If you submit the same address twice, we simply tell you that you are already on the list.
Why we process this information
Survey answers help us understand what actually makes returning to the dentist difficult, so we can decide what to build first. They are used together, as a whole, rather than being read as individual profiles.
Email addresses are used to let you know when the first version of Lavuna is ready to try, and to answer you if you contact us. We do not sell or share them for marketing, and we do not share them with dental practices.
Our lawful basis for processing
For early-access emails we rely on your consent, given when you submit the form. You can withdraw it at any time and we will remove your address.
Anonymous survey answers do not identify anyone, so data protection law does not treat them as personal data. Where any part of that processing does involve personal data, we rely on our legitimate interest in researching and developing the service.
How long we keep information
We keep anonymous survey responses for as long as they remain useful for developing Lavuna: [TO CONFIRM: retention period].
We keep early-access email addresses until the launch contact has been sent and you no longer want to hear from us, or until you ask us to remove your address, and in any case no longer than [TO CONFIRM: retention period].
Service providers who process data on our behalf
We use third-party providers to host this site and store the information described above: [TO CONFIRM: hosting and database provider] and [TO CONFIRM: email provider, if used]. They process data on our instructions only. Information may be stored in [TO CONFIRM: country or region where data is stored].
Your rights
You have the right to ask what personal data we hold about you, to have it corrected or deleted, to object to or restrict how we use it, to receive a copy of it, and to withdraw consent. Because survey responses are anonymous, these rights apply in practice to early-access email addresses.
You can also complain to your data protection regulator. In the UK that is the Information Commissioner's Office.
How to request deletion of your early-access email
Email info@lavuna.com from the address you signed up with, or use the unsubscribe link in any message we send, and we will remove your address. No explanation is needed.

Cookies and analytics
This site does not set advertising cookies and does not use advertising trackers. Our hosting provider may record standard technical request information, such as the time of a request, for security and reliability.
